Compassionate Care Coordinator

Role Overview: Hunter AHP Resourcing seeks a skilled Social Worker to support adults with complex needs in a Glasgow rehabilitation setting. As part of our multidisciplinary team, you will facilitate discharge planning and provide intervention for safeguarding concerns and crisis situations.
About the Position: In this challenging yet rewarding role, you will collaborate with healthcare professionals, community teams, and external agencies to ensure seamless care coordination and high-quality service delivery.
Your Skills and Experience: To succeed in this position, you must possess a strong understanding of adult legislation, such as the Adult Support and Protection (Scotland) Act 2007 and Adults With Incapacity (Scotland) Act 2000. Additionally, you should have experience of facilitating discharge of Older Adults from hospital settings.
What We Offer:
- A competitive hourly rate of £31-35 per hour.
- Flexible working hours (Monday to Friday).
- Contract duration: 3-6 months.
