Job Description:

We are seeking an exceptional Clinical Lead to join our multidisciplinary team supporting an expanding Frailty service. As a key member of our team, you will be responsible for providing comprehensive geriatric assessments of frail elderly patients.

You will work closely with colleagues to ensure seamless delivery of clinical care, participation in MDT meetings, and assessment of patients at risk of admission via 'hot clinics.'

The successful candidate will have strong leadership skills, excellent communication abilities, and a passion for delivering high-quality patient care.
