About the Job

This role involves supporting individuals to stay connected to their local communities, engage in their interests and hobbies, and access peer support. A car is preferred, but not essential.

Key responsibilities include:

  • Supporting individuals with dementia to live independently
  • Engaging individuals in activities that promote social inclusion and well-being

Estimated salary: £28,000 - £32,000 per annum.
