Hospice at Home Health Care Support Worker Position

We are recruiting for a dedicated End of Life Support Worker to join our Transformation Unit team.

**Annual Salary:** £24,071 - £25,674

Job Summary

This is a highly rewarding opportunity to provide compassionate care to patients in their own homes as part of our Hospice at Home service.

Key Responsibilities
  1. Work under the supervision of a senior nurse, delivering high-quality nursing care to patients in their own environments.
  2. Develop strong relationships with patients and their families/carers, providing emotional support and practical care.
  3. Maintain accurate records and adhere to individualised care plans.
  4. Foster a safe and respectful working environment, promoting patient dignity and equality.
  5. Collaborate with colleagues to achieve excellent patient outcomes.
Essential Skills and Qualifications
  • Effective communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Basic knowledge of end-of-life symptoms and physiology
  • Proficiency in Electronic Patient Records (EPR)
Desirable Qualifications and Experience
  • Current UK driving license
  • Level 3 Care Certificate
  • Strong numerical and literacy skills
Working Conditions

The successful candidate will be required to work flexibly across all Trust locations to meet service demands. COVID-19 vaccination is strongly recommended to protect yourself and our patients from the virus when working in healthcare settings.
