Job Title: Healthcare Support Worker for Palliative Care
Salary: £25,000 - £30,000 per annum (dependent on experience)
About the Role
We are seeking an experienced Healthcare Support Worker to join our team in providing palliative care services to patients in their own homes. As a key member of our Hospice at Home team, you will be responsible for delivering high-quality end-of-life care, working closely with patients, families, and healthcare professionals.
Key Responsibilities
Deliver personalized care and support to patients with complex needs, promoting dignity and respect throughout their care journey.Work collaboratively with multidisciplinary teams to ensure seamless care delivery and effective communication.Provide education and training to patients and their families on palliative care, symptom management, and advance care planning.
Requirements
To be successful in this role, you will require:
A recognized qualification in healthcare or a related field (e.g., Level 3 Diploma in Adult Care).Proven experience in delivering palliative care services, preferably within a community or hospice setting.Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and colleagues.
