Job SummaryThe successful candidate will be responsible for delivering clinical and therapeutic care to patients, performing patient assessments, and developing care plans in collaboration with the multidisciplinary team. You will also be expected to provide health education, promote independence, and assist patients with daily living activities, ensuring their dignity and privacy are maintained at all times.
Main Responsibilities- Deliver high-quality care and support to patients in their homes, promoting independence and enhancing their overall well-being.
- Perform patient assessments, develop care plans, and provide health education to patients and their families.
- Assist patients with daily living activities, ensuring their dignity and privacy are maintained at all times.
- Work collaboratively with the multidisciplinary team to ensure seamless care and support.
- Participate in clinical supervision, in-service training, and individual performance development processes.
- Written, verbal, and numeric skills to GCSE or similar level.
- NVQ Level 3 health-related study or equivalent experience.
- Experience of working in a caring role, preferably in a healthcare setting.
- Ability to perform core clinical competencies and demonstrate initiative following minimal direction.
- Good written, spoken, and non-verbal communication skills.
The post holder may be required to work at any of the Trust's sites in line with service needs. The successful candidate will be expected to work flexibly, including evenings and weekends, to meet the needs of the service.
