Senior Community Nurse Specialist

The Essex Partnership University NHS Foundation Trust is seeking a highly skilled and dedicated Senior Community Nurse to join our Specialist Dementia and Frailty Team. As a key member of our multidisciplinary team, you will be responsible for providing high-quality, evidence-based care to patients with dementia and frailty, working collaboratively with healthcare professionals, carers, and service users to deliver a comprehensive assessment, diagnostic, and treatment pathway.
About the RoleThis is a challenging and rewarding role that requires a strong understanding of mental health legislation, principles of CPA and Care Management, Mental Capacity Act, Mental Health Act 1983, and safeguarding. You will be responsible for assessing, planning, implementing, and evaluating packages of care based on individual service user needs, under the guidance of the Advanced Nurse Practitioner. You will also have the opportunity to work autonomously and as part of a team, utilizing your excellent communication and interpersonal skills to deliver the highest level of patient care and service delivery improvements.
Key Responsibilities- Conduct assessments and reviews of care in line with mental health care clusters
- Develop and implement care plans based on individual service user needs
- Work collaboratively with healthcare professionals, carers, and service users to deliver a comprehensive assessment, diagnostic, and treatment pathway
- Supervise and manage junior team members within your professional group and team locality
- Participate in multidisciplinary team meetings and contribute to the development of care plans
We are looking for a Registered Mental Health Nurse (RMN) with a strong understanding of mental health legislation, principles of CPA and Care Management, Mental Capacity Act, Mental Health Act 1983, and safeguarding. You will have experience in the performance management and supervision of lower-banded staff and excellent IT skills, including Word, Excel, PowerPoint, and Outlook. A full UK driving license and access to a car are essential for this role.
