About the Role

We are seeking a highly skilled and dedicated Registered Mental Health Nurse (RMN) to join our Specialist Dementia and Frailty Team. As a key member of the team, you will be responsible for providing high-quality, evidence-based care to patients with dementia and frailty, working closely with other healthcare professionals to ensure comprehensive assessments, diagnostic, and treatment pathways.

Key Responsibilities
  • Conduct assessments and reviews of care in line with mental health care clusters
  • Develop and implement care plans in collaboration with patients, families, and other healthcare professionals
  • Provide education and support to patients, families, and carers on dementia and frailty care
  • Work collaboratively with other healthcare professionals to ensure seamless care transitions
  • Participate in multidisciplinary team meetings to discuss patient care and share best practices
Requirements
  • Registered Mental Health Nurse (RMN) qualification
  • Experience in dementia and frailty care
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
  • Full UK driving license and access to a car
