We are seeking a highly skilled and experienced Senior Community Nurse to join our Specialist Dementia and Frailty Team. As a key member of our team, you will be responsible for providing high-quality, evidence-based care to patients with dementia and frailty.
Key Responsibilities- Conduct comprehensive assessments and develop individualized care plans in collaboration with patients, families, and healthcare teams.
- Provide expert care and support to patients with dementia and frailty, including medication management, wound care, and palliative care.
- Supervise and mentor junior team members, promoting a culture of excellence and continuous learning.
- Collaborate with multidisciplinary teams to ensure seamless care transitions and optimal patient outcomes.
- Participate in quality improvement initiatives and contribute to the development of best practices in dementia and frailty care.
- Registered Mental Health Nurse (RMN) with a strong background in dementia and frailty care.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Proven experience in assessment, care planning, and medication management.
- Ability to work autonomously and as part of a team, with a strong commitment to continuous learning and professional development.
- Full UK driving license and access to a car for work purposes.
