Senior Community Nurse

We are seeking a highly skilled and dedicated Senior Community Nurse to join our team at Essex Partnership University NHS Foundation Trust. As a Senior Community Nurse, you will play a vital role in providing high-quality, patient-centered care to individuals in their own homes and communities.
Key Responsibilities- Conduct comprehensive assessments and develop personalized care plans to meet the unique needs of each patient.
- Work collaboratively with multidisciplinary teams, including healthcare professionals, social workers, and other stakeholders, to ensure seamless care coordination.
- Provide education and support to patients and their families to promote health and well-being.
- Participate in quality improvement initiatives to enhance patient outcomes and experiences.
- Supervise and mentor junior staff members to ensure the delivery of high-quality care.
- Registered Mental Health Nurse (RMN) qualification.
- Experience in community nursing, preferably in a mental health setting.
- Excellent communication and interpersonal skills.
- Ability to work autonomously and as part of a team.
- Full UK driving license and access to a car.
