Job Summary

The Essex Partnership University NHS Foundation Trust is seeking a highly skilled and dedicated Senior Community Nurse to join our Specialist Dementia and Frailty Team. As a key member of our team, you will be responsible for providing high-quality, evidence-based care to patients with dementia and frailty, working collaboratively with multidisciplinary teams to deliver comprehensive assessments, diagnostic, and treatment pathways.

About the Role

This is a challenging and rewarding role that requires a strong understanding of mental health legislation, principles of CPA and Care Management, Mental Capacity Act, Mental Health Act 1983, and safeguarding. You will be responsible for assessing, planning, implementing, and evaluating packages of care based on individual service user needs, under the guidance of the Advanced Nurse Practitioner.

Key Responsibilities
  • Conduct comprehensive assessments and reviews of care in line with mental health care clusters.
  • Develop and implement individualized care plans that meet the unique needs of each service user.
  • Work collaboratively with multidisciplinary teams to deliver comprehensive assessments, diagnostic, and treatment pathways.
  • Supervise and manage junior team members, providing guidance and support to ensure high-quality care delivery.
  • Participate in performance management and supervision of lower-banded staff, ensuring adherence to Trust policies and procedures.
Requirements
  • Registered Mental Health Nurse (RMN) qualification.
  • Experience in performance management and supervision of lower-banded staff.
  • Strong understanding of health legislation, principles of CPA and Care Management, Mental Capacity Act, Mental Health Act 1983, and safeguarding.
  • Excellent communication and interpersonal skills, with the ability to work effectively with multidisciplinary teams.
  • Full UK driving license and access to a car for work purposes.
