About the Role

We are seeking a highly skilled and experienced Registered Mental Health Nurse (RMN) to join our Specialist Dementia and Frailty Team as a Senior Community Nurse. This is an exciting opportunity to work in a dynamic and supportive environment, providing high-quality care to patients with dementia and frailty.

Key Responsibilities
  • Conduct comprehensive assessments and reviews of care in line with mental health care clusters.
  • Develop and implement packages of care based on individual service user needs, under the guidance of the Advanced Nurse Practitioner.
  • Supervise and manage junior team members within the professional group and team locality.
  • Collaborate with key partner organisations, service users, carers, clinicians, and other practitioners to deliver services.
Requirements
  • Registered Mental Health Nurse (RMN) qualification.
  • Experience in performance management and supervision of lower-banded staff.
  • Understanding of health legislation, principles of CPA and Care Management, Mental Capacity Act, Mental Health Act 1983, and safeguarding.
  • Excellent IT skills, including Word, Excel, PowerPoint, and Outlook.
  • Flexible approach to work and ability to travel across sites and Trust boundaries.
