About the Role

We are seeking a highly skilled and compassionate Registered Mental Health Nurse (RMN) to join our Specialist Dementia and Frailty Team as a Senior Community Nurse Specialist.

As a key member of our team, you will be responsible for providing high-quality, evidence-based care to patients with dementia and frailty, working closely with our multidisciplinary team to deliver a comprehensive assessment, diagnostic, and treatment pathway.

Key responsibilities will include assessing, planning, implementing, and evaluating packages of care based on individual service user needs, under the guidance of the Advanced Nurse Practitioner.

You will also have the opportunity to work collaboratively with key partner organisations, service users, carers, clinicians, and other practitioners to deliver services, providing a 7-day a week, 24-hours a day service for 365 days a year.

Requirements

  • Registered Mental Health Nurse (RMN) qualification
  • Experience in the performance management and supervision of lower-banded staff
  • Understanding of health legislation, principles of CPA and Care Management, Mental Capacity Act, Mental Health Act 1983, and safeguarding
  • Excellent IT skills, including Word, Excel, PowerPoint, and Outlook
  • Flexible approach to work and ability to travel across sites and Trust boundaries
