Job Title: Senior Community Nurse for Children with Disabilities

We are seeking a highly skilled and compassionate Senior Community Nurse to join our team at Transformation Unit. As a Senior Community Nurse, you will play a vital role in providing high-quality, evidence-based care to children with disabilities.

About the Role

This is a challenging and rewarding role that requires a strong understanding of mental health care clusters, health legislation, and risk assessment management. You will work closely with our Advanced Nurse Practitioner to assess, plan, implement, and evaluate packages of care for patients with dementia and frailty.

As a Senior Community Nurse, you will also have responsibility for supervising and managing junior team members within your professional group and team locality.

Key Responsibilities
  • Conduct assessments and reviews of care in line with mental health care clusters
  • Develop and implement care plans that meet the individual needs of patients
  • Supervise and manage junior team members
  • Collaborate with other healthcare professionals to ensure seamless care delivery
Requirements
  • Registered Mental Health Nurse (RMN) qualification
  • Experience in performance management and supervision of lower-banded staff
  • Full UK driving license and access to a car for work purposes
