About the Role

We are seeking a highly skilled and compassionate Registered Mental Health Nurse to join our Specialist Dementia and Frailty Team. As a key member of our team, you will be responsible for providing high-quality, evidence-based care to patients with dementia and frailty.

Key Responsibilities
  • Conduct comprehensive assessments and develop individualized care plans in collaboration with the Advanced Nurse Practitioner.
  • Provide holistic care to patients, addressing their physical, emotional, and social needs.
  • Supervise and manage junior team members, promoting a culture of learning and development.
  • Collaborate with multidisciplinary teams to ensure seamless care coordination and patient outcomes.
Requirements
  • Registered Mental Health Nurse (RMN) qualification.
  • Experience in performance management and supervision of lower-banded staff.
  • Understanding of health legislation, principles of CPA and Care Management, Mental Capacity Act, and safeguarding.
  • Full UK driving license and access to a car for work purposes.
