Clinical Nurse Specialist

We are seeking a highly skilled and experienced Registered Nurse to join our Physical Health Team as a Band 6 Clinical Nurse Specialist. The successful candidate will work closely with the team to deliver high-quality physical health assessments to individuals with severe mental illness.
Key Responsibilities- Provide expert clinical support to junior colleagues and contribute to the assessment of physical and mental health needs.
- Develop and implement effective care plans to improve the physical health of individuals with severe mental illness.
- Collaborate with primary care professionals, Primary Care Networks, and wider health and social care services to promote the inclusion of individuals with severe mental illness into mainstream services.
- Support audit and data collection for service improvement and contribute to the development of policies and procedures.
- Provide practice supervision and support for nursing students and student nursing associates on placement.
- Liaise directly with GPs, practice nurses, and other primary care staff to ensure seamless care delivery.
- Registered Nurse - Mental Health/Learning disability/General with current registration with the NMC.
- ENB 998 / Preceptorship and/or Practice Supervisor and Assessor Training.
- At least 3 years' experience post registration at Band 5 and community nursing experience.
- Detailed awareness of relevant evidence base relating to physical health in SMI population.
