Community Health Nurse

We are seeking a highly skilled and dedicated Community Nurse to join our team at Cambridgeshire and Peterborough NHS Foundation Trust. As a Community Nurse, you will play a vital role in providing high-quality care to patients in their own homes, working closely with other healthcare professionals to ensure seamless care delivery.
Key Responsibilities- Assess, plan, implement, and evaluate patient care in the community setting
- Work collaboratively with multidisciplinary teams to ensure comprehensive care delivery
- Provide education and support to patients, carers, and other healthcare professionals
- Contribute to the development and implementation of care plans that meet individual patient needs
- Participate in quality improvement initiatives to enhance patient outcomes and care delivery
- Registered General Nurse (RGN) qualification and current registration with the NMC
- Experience of working in the community setting, preferably with a focus on primary care
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, carers, and other healthcare professionals
- Ability to work autonomously and as part of a team, with a flexible approach to working hours
- Knowledge of relevant legislation, policies, and guidelines that impact community nursing practice
