About the Community Nurse Role


We are seeking an experienced Community Nurse to join our team in Cambridgeshire and Peterborough NHS Foundation Trust.



Main Responsibilities

  • Work as an autonomous practitioner with the integrated multi-professional team
  • Be accountable for assessing, interpreting, planning, implementing and evaluating treatment to patients within professional guidelines
  • Keep the patient at the centre of care, ensuring patient and carer participation in decision-making
  • Provide support and education to peers, new staff, non-registered staff, and students
  • Provide high standards of nursing intervention within patients' own homes, including lone working, with access to specialist and advanced practitioners when required


Requirements

  • Experience of working in the community
  • Essential qualifications: RGN level 1 and RGN level 2
  • Desirable qualifications: Mentorship qualification or equivalent, Management of leg ulcer qualification
  • Ability to work flexibly as part of a team, good documentation skills, and ability to assess, plan and implement care and formulate care plans
  • Ability to safeguard and promote the welfare of children and young people/vulnerable adults
  • Good communication skills and appreciation of the significance of safeguarding
