Job Overview
We are seeking a skilled Community Nurse to join our St Neots Team. As a Band Community Nurse, you will work closely with our multidisciplinary team to provide excellent nursing care at home, reducing the need for acute hospital admission.
Key Responsibilities:
- Provide high-quality nursing care to patients in their own homes
- Work collaboratively with colleagues, including general practitioners and other healthcare professionals
- Assess and formulate care plans to meet individual patient needs
- Teach and support student Nurses and Health Care Assistants
- Contribute to a culture of high-quality care and service provision
Requirements:
- Experience of working in the Community
- Education to RGN level
- Commitment to high-quality care and service provision
- Able to work flexibly and as part of a team
- Ability to assess and formulate care plans
- Enhanced communication and inter-personal skills
