About the Role

Division of Surgery – BHRUT NHS is seeking a highly skilled Patient Admission Pathway Co-ordinator to support our innovative team in reducing emergency hospital admissions and reliance on acute hospital care.

Key Responsibilities

  • Monitor patients and conduct holistic needs assessments using recognised assessment tools and remote technology solutions.
  • Collaborate with professional leads, GP practices, and local Primary Care Networks to develop services that meet local needs.
  • Work independently in the community, conducting clinical assessments and developing care plans.

Requirements

  • Diploma or Degree in Adult Nursing.
  • Current registration and evidence of current CPD.
  • Minimum 2 years experience post registration.
