**Job Description:**

  • Complete psychosocial assessments and develop plans of care in collaboration with patients and caregivers.
  • Nurture professional relationships with community partners to coordinate services.
  • Attend meetings and conferences for professional development and coordination.
  • Supervise students and interns as deemed beneficial.

**Requirements:**

  • Master's Degree in Social Work from a recognized University program.
  • Current registration with the Ontario College of Social Workers and Social Service Workers.
  • Excellent communication and computer skills.

**Estimated Salary:** $65,000 - $85,000 per year.
