About the Role:
We are seeking a skilled and compassionate Nurse Practitioner to join our multi-discipline team in a part-time capacity.
As a key member of our team, you will be responsible for providing high-quality patient care, developing treatment plans, and collaborating with other healthcare professionals to ensure comprehensive care.
Key Responsibilities:
- Patient Assessment & Diagnosis:
- Conduct thorough health assessments, including physical examinations, history taking, and diagnostic tests.
- Formulate accurate differential diagnoses based on clinical findings and patient history.
- Order, perform, and interpret diagnostic tests and imaging as required.
- Treatment & Management:
- Develop, implement, and evaluate evidence-based treatment plans.
- Prescribe medications, therapies, and other interventions within scope of practice as regulated by the College of Nurses of Ontario (CNO).
- Provide education to patients and families regarding management of health conditions, treatment plans, and wellness goals.
- Health Promotion & Disease Prevention:
- Offer guidance and resources for disease prevention, health promotion, and lifestyle changes.
- Conduct screenings and immunizations for individuals across all age groups.
- Address mental health needs, offering counseling or referrals as appropriate.
- Collaboration & Communication:
- Collaborate with other healthcare professionals to coordinate comprehensive care plans.
- Provide consultation, education, and mentorship to other healthcare team members as needed.
- Communicate effectively with patients, families, and the healthcare team to ensure continuity of care.
- Documentation & Record-Keeping:
- Maintain accurate, up-to-date patient records using electronic medical records (EMR) systems.
- Document patient interactions, diagnoses, treatment plans, and follow-up care in compliance with legal and ethical standards.
- Professional Development & Continuous Learning:
- Stay current with the latest clinical guidelines, best practices, and medical research relevant to the role.
- Attend professional development opportunities and maintain certification/licensure requirements as set by the College of Nurses of Ontario.
