Palliative Care Professional

This position is ideal for a compassionate individual who values hospice palliative care and is eager to support our Hospice organization and a collaborative team.
The successful candidate will be dedicated to the values, mission, and vision of the Hospice of Windsor and Essex County within the organization and the community.
Key Responsibilities- Conduct psychosocial assessments and develop plans of care in collaboration with patients and caregivers.
- Develop and nurture professional relationships with community partners.
- Attend meetings and conferences for coordination and professional development purposes.
Requirements:
- Technical Requirements
- Current registration with the Ontario College of Social Workers and Social Service Workers.
- Excellent communication and computer skills.
- A valid driver's license and willingness to work flexible hours.
- A Master of Social Work Degree from a recognized University program.
Compensation: A competitive salary based on experience, estimated to be around $60,000 - $80,000 per year.
