Job Description:

A comprehensive care navigator is sought to join our multidisciplinary healthcare team in Windsor, Berkshire.

We are part of the Maidenhead Primary Care Network, a group of 6 GP Practices working together with local providers to deliver coordinated health and social care services.

The successful candidate will play a key role in proactively identifying individuals' care and support needs, including those with long-term conditions or frailty, and coordinating navigation of care and support across health and social care services.

They will work closely with GPs, practice teams, and other professionals to ensure that appropriate support is made available, enabling individuals to understand and manage their condition and address changing needs.

The post holder will be based in a local cluster of General Practices and will be caring, dedicated, reliable, and person-focused. They will enjoy working with a diverse range of people, have excellent communication skills, and strong organisational and time management skills.

This is an integral part of our multidisciplinary team, working alongside social prescribing link workers and health and wellbeing coaches to provide an all-encompassing approach to personalised care and promote embedding personalised care approaches across the network.

Estimated salary: £28,000 - £38,000 per annum depending on experience.
