London, Greater London, United Kingdom Cambridgeshire County Council Full time

About the Role
We are seeking a Person-Centred Health Support Specialist to join our High Impact Users (HIU) service, which aims to improve the outcomes of individuals frequently using NHS urgent and emergency care services. The successful candidate will work with a county-wide focus, mainly in people's homes, cafés, community venues, or anywhere the individual feels comfortable. This is a flexible role, with the possibility to complete admin work from home or in a range of Cambridgeshire County Council office spaces.

About the Service
The HIU service is hosted by Cambridgeshire County Council on behalf of the wider Integrated Care System and will trial a more proactive and personalised approach. Our team is passionate about delivering high-quality care and support to individuals who need it most.

About You
You will have a good understanding of health inequalities and the wider determinants of health. You will have significant experience as a case holding practitioner/worker, with an approach that aligns with personalised care. You will also have a coaching qualification, or have trained in motivational interviewing, appreciative enquiry or similar. Experience of partnership working and navigating services is also essential.

About Us
Cambridgeshire County Council is a great place to work and live, with a diverse population, an urban centre in Cambridge, historic Isle of Ely, Huntingdon and St Ives, as well as more rural landscapes. We are proud to serve the diverse communities of Cambridgeshire and want our workforce to be reflective of this diversity, which we believe benefits our employees, the organisation, and our communities. We offer a wide range of learning and development opportunities, apprenticeships, our wellbeing package, staff benefits, and a commitment to equality, diversity, and inclusion.
