Young Person Centred Practitioner

Key Responsibilities:
- Providing one-to-one support to young people with complex needs.
- Developing and delivering personalized care plans to meet the individual needs of each young person.
- Collaborating with colleagues to ensure seamless support across all aspects of the young person's life.
Requirements:
- Experience of working with children or young people, preferably with SEND.
- Ability to demonstrate a person-centred approach to working with young people.
- Ability to develop and deliver personalized care plans.
We offer a competitive salary of £27,000 - £30,000 per annum, depending on experience, along with a range of benefits including 40 days annual leave and a generous pension plan.
