Job Description:We are seeking an experienced and dedicated individual to join our team as a Patient Care Coordinator. This role involves delivering high-quality patient care, promoting recovery-focussed care, and supporting patients with their rehabilitation needs.
Main Responsibilities:- Patient Care: Provide high-quality patient care, promoting recovery-focussed care and supporting patients with their rehabilitation needs.
- Communication: Maintain effective communication with patients, colleagues, and other healthcare professionals to ensure seamless care.
- Confidentiality: Understand and adhere to patient confidentiality guidelines, maintaining the trust and respect of patients and their families.
The successful candidate can expect a salary of £23,615 per annum, which is highly competitive in the industry.
Location:This role is based at our Western Community Hospital, providing a unique opportunity to work in a specialist rehabilitation unit.
