This role involves working closely with a multidisciplinary team to provide high-quality care and support services. You will be responsible for developing and implementing effective care plans, maintaining accurate records, and collaborating with other professionals to ensure seamless service delivery.

The estimated salary for this position is £51,376 per annum, based on 40 hours per week at £12.80 per hour.

Responsibilities:

  • Develop and implement effective care plans for children and young people with learning disabilities and mental health needs
  • Work collaboratively with a multidisciplinary team to provide high-quality care and support services
  • Maintain accurate records and reports related to the care and support provided
  • Support children and young people in achieving their goals and aspirations

Requirements:

  • Degree in a relevant field (e.g., social work, psychology, education)
  • Level 3 qualification in children and young people or equivalent
  • Experience working with children and young people with learning disabilities and mental health needs
  • Excellent communication and interpersonal skills
