Support Team Coordinator

Avenues Group is dedicated to supporting young adults and adults with learning disabilities, autism and epilepsy through home-based and community-based care.
Job RoleThe successful candidate will be responsible for motivating and inspiring the team to deliver high-quality care and support as a part-time Care Support Manager in Chapelmede, Horley, with a competitive salary of £27,154 per annum.
Key Tasks:- Attend appointments with individuals we support, attend care reviews, review and update care plans and risk assessments, record and monitor daily logs, lead shifts and administer medication.
- Provide day-to-day support for individuals we are supporting.
- Experience in supporting individuals with disabilities and complex needs.
- Ability to support individuals with their personal care.
- Flexibility to work across different shift patterns on a 7-day rota.
