As a Nurse Practitioner in Community Settings, you will play a critical role in providing high-quality patient care in the comfort of their own homes. Kent Community Health NHS Foundation Trust is an outstanding organisation, rated by the CQC, and we are committed to delivering exceptional patient care and improving the health of our communities.
The estimated salary for this position is £39,622 - £50,064 per annum, depending on experience. We offer flexible working arrangements, allowing you to work in a way that suits you, your life, and your family. Our service operates seven days a week from 8am - 10pm, ensuring that you can maintain a healthy work-life balance.
Your primary responsibilities will include:
- Assessing the healthcare needs of patients and developing holistic care plans
- Providing complex nursing care to people at home and in residential care homes
- Working collaboratively with other healthcare professionals to ensure seamless care delivery
