Community Care Specialist

At Kent Community Health NHS Foundation Trust, we are committed to delivering exceptional patient care and improving the health of our communities. As a Community Care Specialist, you will play a vital role in providing complex nursing care to people at home and in residential care homes.
The estimated salary for this position is £32,000 - £43,041 per annum, depending on experience. We offer a flexible working arrangement that allows you to work in a way that suits you, your life, and your family. Our service operates seven days a week from 8am - 10pm, ensuring that you can maintain a healthy work-life balance.
Your primary responsibilities will include:
- Assessing the healthcare needs of patients and families
- Monitoring the quality of care they receive
- Being professionally accountable for its delivery
You will be part of a close-knit team who are dedicated to the health and wellbeing of people in Faversham. Our community nursing teams are closely aligned with local Primary Care Networks, ensuring that patient care is holistic and joined up.
