Individual Care Coordinator Role

We are seeking an experienced Individual Care Coordinator to join our team at The Sycamores. As a Night Support Worker, you will play a vital role in delivering exceptional care and support to individuals with mental health needs.

About Our Service:

The Sycamores is a four-bed community rehabilitation (level 1) residential service for men who have progressed through hospital pathways. Our goal is to provide a safe, stimulating, and trusting environment that empowers individuals to achieve their full potential.

Your Key Responsibilities:

  • Develop and implement personalized care plans, ensuring ongoing evaluation and improvement
  • Work collaboratively with multidisciplinary teams to achieve positive outcomes
  • Deliver tailored care and support to meet individual needs
  • Ensure a safe and clean environment, adhering to health and safety standards
