**Role Description**

This is a full-time Care Coordinator role located on-site in East Molesey. As a Care Coordinator, you will be responsible for coordinating all aspects of care delivery to ensure our clients receive compassionate, effective, and safe care in their homes. Your duties will include conducting initial assessments, arranging, and scheduling care services, coordinating with healthcare providers, tracking client progress, maintaining accurate records, and providing excellent customer service to clients and their families.

**Qualifications**
- Care Coordination and Case Management skills
- Excellent Communication skills
- Experience in Home Care or related field
- Customer Service skills with an ability to build rapport with clients and their families.
- Strong organizational skills and attention to detail
- Excellent problem-solving and critical thinking skills
- Experience and proficiency in Microsoft Office and other relevant software/tools
- Bachelor's degree in related field preferred but not required.
- Certification in care coordination or case management preferred but not required.

**Salary**: £25,000.00-£30,000.00 per year
