About the Role:

We are seeking a skilled and compassionate Rehabilitation Assistant to join our team at Active Care Group. As a Rehabilitation Assistant, you will play a vital role in supporting individuals with mental health conditions to achieve their goals and live fulfilling lives.

Key Responsibilities:

  • Build strong relationships with service users, promoting open and honest communication.
  • Support individuals to undertake their rehabilitation programs, promoting independence and confidence.
  • Establish effective methods of communication, reducing barriers when necessary.
  • Assist with activities of daily living and personal care, ensuring the well-being of service users.
  • Communicate any issues to the clinical team, ensuring seamless care.
  • Accurately record and report work activities, maintaining high standards of documentation.

Requirements:

  • A genuine interest in providing person-centered care and support.

  • A successful Enhanced DBS check will be required for this role.
