We are seeking a dedicated and compassionate Rehabilitation Assistant to join our team at Blackburn Road in Lancashire.

As a Rehabilitation Assistant, you will play a vital role in supporting individuals with diagnosed mental health conditions to achieve their goals and live fulfilling lives.

Key Responsibilities:

  • Build meaningful relationships with service users, fostering open and honest communication.
  • Support individuals to undertake their rehabilitation programme, promoting independence and empowerment.
  • Establish effective methods of communication, reducing barriers and ensuring seamless support.
  • Assist with activities of daily living and personal care, prioritizing dignity and respect.
  • Communicate effectively with the clinical team, ensuring seamless care coordination.
  • Accurately record and report work activities, maintaining high standards of documentation.
