About the Role

We are seeking a skilled and compassionate Rehabilitation Assistant to join our team at Active Care Group. As a key member of our team, you will play a vital role in supporting individuals with mental health conditions to achieve their goals and live fulfilling lives.

Key Responsibilities
  • Build strong relationships with service users, promoting open and honest communication
  • Support individuals to undertake their rehabilitation programmes, promoting independence and confidence
  • Establish effective methods of communication, reducing barriers and promoting understanding
  • Provide support with activities of daily living and personal care, ensuring dignity and respect
  • Communicate effectively with the clinical team, sharing information and concerns
  • Accurately record and report work activities, maintaining high standards of documentation

Requirements
  • A genuine interest in providing person-centred care and support
  • Effective communication and interpersonal skills, with the ability to work with diverse individuals
  • Experience working in the care sector or mental health, although not essential
  • Successful candidates will be required to undergo an Enhanced DBS check
