About the Role

We are seeking a qualified Mental Health Social Worker to join our Supported Accommodation Team in Hampshire. As a key member of our team, you will work closely with our Community Teams and Commissioning Team to address complex issues affecting individuals with learning disabilities and autism.

Responsibilities include facilitating hospital discharges from mental health settings, attending discharge planning meetings, and compiling social circumstances reports for courts. Our supportive team environment enables you to excel in this demanding yet rewarding role.

Requirements
  • A strong background in mental health social work, with experience working with individuals with learning disabilities and/or autism.
  • Excellent relationship management skills, with the ability to build positive relationships with individuals, families, carers, and other professionals.
  • Innovative problem-solving skills, with a commitment to delivering person-centred support.
