We are seeking a highly skilled and compassionate Independent Living Specialist to join our team at Pin Point Health & Social Care. As a key member of our community care team, you will be responsible for delivering high-quality support services to individuals with diverse needs and aspirations.

Responsibilities:

* Deliver person-centred support plans tailored to each Service User's unique needs and goals

* Conduct risk assessments and develop strategies to mitigate risks and promote independence

* Liaise with external professionals to inform and develop Service Users' support packages

* Provide emotional support and encouragement to empower Service Users to make informed decisions about their lives

* Maintain accurate and up-to-date records of Service User interactions and progress

* Participate in regular team meetings and contribute to the development of best practices

* Assist Service Managers in planning, negotiating, and managing budgets within organisational financial policies and procedures

* Represent Pin Point Health & Social Care positively and professionally in all interactions with Service Users, colleagues, and stakeholders

Requirements:

* Experience working with individuals with diverse needs and aspirations

* Ability to work independently and as part of a team

* Excellent communication and interpersonal skills

* Knowledge of relevant legislation, organisational, and local policies and procedures

* Willingness to undertake relevant personal development and training
