We are seeking an experienced Social Worker to join our Disabled Children's Team in Kings Hill. As a key member of our team, you will work across West Kent to assess needs, develop care plans, and deliver effective interventions.

Key Responsibilities:

  • Conduct in-depth assessments of children with disabilities and their families.
  • Develop, implement, and review personalized care plans that address individual needs and circumstances.
  • Work collaboratively with multidisciplinary teams, including healthcare providers, educators, and community organizations.
  • Provide direct support, advocacy, and guidance for children and families.
  • Maintain detailed and accurate case records in compliance with legal and organizational standards.
  • Offer mentorship and support to newly qualified social workers and other team members.
  • Engage in regular supervision and ongoing professional development to stay current with best practices and policy changes.
