We are seeking an experienced and passionate Social Worker to join our Disabled Children's Team in Kings Hill. As a key member of our team, you will play a vital role in supporting disabled children and their families across West Kent.
Key Responsibilities:
- Conduct comprehensive assessments of children with disabilities and their families to identify their needs and develop personalized care plans.
- Work collaboratively with multi-disciplinary teams, including healthcare providers, educators, and community organizations, to deliver effective interventions and support.
- Provide direct support, advocacy, and guidance to children and families, promoting their well-being and empowerment.
- Maintain accurate and detailed case records, adhering to legal and organizational standards.
- Offer mentorship and support to newly qualified social workers and other team members, contributing to their professional development.
- Engage in regular supervision and ongoing professional development to stay current with best practices and policy changes.
