We are seeking an experienced Social Worker to join our Disabled Children's Team in Kings Hill. As a key member of our team, you will play a crucial role in assessing needs, developing and implementing care plans, and delivering effective interventions.
Key Responsibilities:- Conduct comprehensive assessments of children with disabilities and their families to identify their unique needs and circumstances.
- Develop and implement personalized care plans that address individual needs and circumstances, working collaboratively with multi-disciplinary teams.
- Provide direct support, advocacy, and guidance for children and families, maintaining detailed and accurate case records in compliance with legal and organisational standards.
- Offer mentorship and support to newly qualified social workers and other team members, engaging in regular supervision and ongoing professional development to stay current with best practices and policy changes.
