About the Role

The East London NHS Foundation Trust is seeking a highly motivated and experienced District Nursing Specialist Practitioner to join our Community Nursing team as a Team Lead. This is an exciting opportunity for a skilled nurse to take on a leadership role and contribute to the delivery of high-quality patient care in the community.

Key Responsibilities

  • Manage a caseload of patients and provide clinical leadership to a team of skilled healthcare workers.
  • Develop and implement effective care plans to meet the needs of patients and their families.
  • Collaborate with other healthcare professionals to ensure seamless care delivery and improve patient outcomes.
  • Provide education and support to patients and their families to promote health and well-being.
  • Participate in the development and implementation of policies and procedures to ensure high-quality patient care.
  • Contribute to the development and implementation of the team's strategic plan to improve patient outcomes and services.

Requirements

  • Hold a specialist practitioner qualification in District Nursing.
  • Have experience of managing a caseload and providing clinical leadership.
  • Demonstrate evidence-based practice and up-to-date clinical skills.
  • Have a post-registration qualification in a relevant clinical field.
  • Have relevant teaching/mentorship qualification and evidence of teaching skills.
  • Have a current driving license and be able to travel across sites and across Bedfordshire to meet patient needs and attend meetings.
