Job Summary
We are seeking a highly motivated and experienced Band 6 Nurse to join our East London NHS Foundation Trust team as a Community Nursing Team Lead. This dynamic role will involve leading a team of skilled healthcare workers in delivering high-quality nursing services to patients in their own homes.
Main Responsibilities
The successful candidate will be responsible for:
- Managing a caseload of patients and coordinating care with other healthcare professionals
- Providing health education and support to patients and carers to enable them to manage their conditions effectively
- Developing and implementing care plans in conjunction with the multidisciplinary team
- Participating in the strategic planning and development of services provided in the community
- Training and mentoring students during placements
- Effective utilization of staff and non-pay resources, including scheduling and ordering systems
Requirements
The ideal candidate will possess:
- A specialist practitioner qualification in District Nursing
- Experience of managing a caseload and leading a team
- Demonstrated evidence-based practice and up-to-date clinical skills
- Significant post-registration nursing experience, including community settings
- Post-registration qualification in a relevant clinical field
- Relevant teaching/mentorship qualification and evidence of teaching skills
- Literate in IT/computer skills
- Good oral communication skills based on fluency in the English language
- A current driving license
