Job Summary

We are seeking a highly motivated and experienced Community Nurse to join our team as a Caseload Holder. This is an exciting opportunity for a dynamic and ambitious nurse to develop their leadership skills and contribute to delivering high-quality nursing services to patients in their own homes.

Main Responsibilities

The successful candidate will be responsible for managing a caseload of patients, providing responsive and high-quality nursing care, and working collaboratively with the community nursing team to achieve excellent patient outcomes. The role will involve working a variety of shifts, including weekends, evenings, and bank holidays, and will require access to a car to carry out service duties.

About the Role

This is a fantastic opportunity for a nurse who is passionate about patient-centered care and is looking to develop their leadership skills. The successful candidate will have the support and mentorship of the Team Lead and will be encouraged to apply for the SPQDN in due course.

Requirements
  • Has Community Nursing experience
  • Has experience of managing a caseload
  • Demonstrate evidence-based practice and up-to-date clinical skills
  • Evidence of significant post-registration nursing experience, including community setting
  • Post-registration qualification in a relevant clinical field
  • Relevant teaching/mentorship qualification and evidence of teaching skills
  • Literate in IT/Computer Skills
  • Good oral communication skills based on fluency in the English language
  • Current driving license
