Job Summary

We are seeking a highly motivated and experienced District Nursing Specialist to join our team at East London NHS Foundation Trust. As a key member of our community nursing team, you will be responsible for delivering high-quality nursing care to patients in their own homes.

Main Responsibilities
  • Manage a caseload of patients and provide comprehensive nursing care, including health education and support.
  • Work collaboratively with other healthcare professionals to ensure seamless care delivery.
  • Participate in the development and implementation of care plans, ensuring patient-centered care.
  • Contribute to the training and mentorship of students and junior staff members.
  • Utilize scheduling and ordering systems to optimize staff and non-pay resources.
Requirements
  • Hold a specialist practitioner qualification in District Nursing.
  • Have experience of managing a caseload and providing community-based nursing care.
  • Demonstrate evidence-based practice and up-to-date clinical skills.
  • Evidence of significant post-registration nursing experience, including community settings.
  • Post-registration qualification in a relevant clinical field.
  • Relevant teaching/mentorship qualification and evidence of teaching skills.
  • Literate in IT/computer skills.
  • Good oral communication skills based on fluency in the English language.
  • Current driving license.
