Job Summary

We are seeking a highly motivated and experienced Band 5 Nurse to join our Community Nursing team at East London NHS Foundation Trust. This is an exciting opportunity for a dynamic and ambitious nurse to develop their leadership skills and contribute to the delivery of high-quality patient care in the community.

Main Responsibilities

  • Lead a caseload of patients and provide high-quality nursing care in the community
  • Develop and implement care plans in collaboration with patients, families, and healthcare teams
  • Contribute to the development and implementation of community nursing services and policies
  • Provide mentorship and support to junior staff members
  • Participate in quality improvement initiatives and contribute to the improvement of patient outcomes

Requirements

  • Current registration with the NMC as a Band 5 Nurse
  • Experience of working in a community nursing setting
  • Ability to lead and manage a caseload of patients
  • Excellent communication and interpersonal skills
  • Ability to work collaboratively as part of a multidisciplinary team
