Palliative Care Nurse

About the Role:
We are seeking a qualified Nurse to work as an Admission and Discharge Coordinator in our Hospice-based multi-disciplinary Specialist Palliative Care In-Patient Unit. The purpose of this role is to ensure a smooth transition for patients as they arrive and when they are discharged from the In-Patient Unit.
You will work alongside the Multi-Disciplinary Team to provide holistic, person-centred care and support. If you are enthusiastic, flexible, and share our values and care about making a difference in the last years of someone's life, we would love to hear from you.
