About the Role

We are seeking a highly skilled Care Coordination Specialist to join our team at Buckinghamshire Healthcare NHS Trust. As a Community Case Manager, you will play a vital role in providing exceptional care to patients in their homes.

The successful candidate will have a strong background in healthcare, with experience working in a variety of core areas such as elderly care, orthopaedics, neurology, and palliative care. You will be responsible for managing a caseload of patients with long-term conditions and implementing strategies to support the team in delivering efficient and safe care.

This is an excellent opportunity to develop your leadership skills and contribute to the delivery of high-quality patient care. You will work closely with colleagues to coordinate visits and manage the administration of holistic care in the community.

In this role, you will have the opportunity to:

  • Assess the physical and psychosocial needs of patients and develop tailored interventions
  • Contribute to processes for proactive case finding and referral management
  • Develop and maintain partnership working with other healthcare professionals

The ideal candidate will have excellent written and verbal communication skills, be able to motivate staff, and demonstrate self-awareness. You will also have a valid and current driving license or be able to travel to locations throughout Buckinghamshire.

Salary: £42,000 - £62,000 per annum (dependent on experience)
