Care Engagement Specialist

We are seeking a highly motivated Care Engagement Specialist to join our team and contribute to delivering exceptional care and engagement services to residents.
Your key responsibilities will include developing and implementing care engagement strategies, working collaboratively with the care team, and maintaining accurate records of programme planning and evaluation.
Main Responsibilities:
- To develop and implement effective care engagement strategies that meet residents' diverse needs and preferences.
- To work closely with the care team to ensure seamless delivery of care services and excellent resident outcomes.
- To maintain accurate records of programme planning, implementation, and evaluation.
Salary Estimate: £22,000 - £27,000 per annum, depending on experience. Part-time contract, 24 hours per week.
