Job Description:
\We are seeking an experienced Service Coordinator to join our team at Job Switch Ltd. The ideal candidate will have a proven track record in hospital discharge work, care Act Assessments, and arranging care.
\Key Responsibilities:

- Develop and maintain relationships with healthcare professionals and local authorities to ensure smooth transitions for service users.\
- Conduct thorough assessments to identify individual needs and develop personalized care plans.\
- Collaborate with internal stakeholders to arrange appropriate care and support services.\
- Maintain accurate records and reports to ensure compliance with regulatory requirements.\
- Stay up-to-date with current healthcare policy and legislation to inform practice.\\/ul">
Requirements:
\The successful candidate will possess excellent communication and interpersonal skills, with the ability to build strong relationships with diverse stakeholders.
\A degree in a relevant field and experience in a similar role are essential. A postgraduate diploma or equivalent qualification in a specialist area would be advantageous.
\Salary: £40,000 - £55,000 per annum, depending on experience.
